1. What other paperwork will you need for my child?
We will need the following forms for your child:

- Completed Registration Form: Click here to download
- Completed Participant Contract: Click here to download
- Completed Sports Medical Release Form: Click here to download
- Copy of Birth Certificate
- Copy of Their FINAL Complete Year Report Card
- Current Wallet Size Photo

2. What is South Valley's Refund Policy?
A refund will be granted ONLY upon injuries that disable an athlete from participating in a registered activity before that activity begins (a $75.00 processing fee will be withheld). A doctor's note must be submitted with a written request from the Parent/Guardian. No other refunds will be granted. Our "NO REFUND POLICY" is to protect the integrity and success of the organization, as cancellations effect everyone involved. Moreover, fluctuating numbers can lead to the cancellation of association events (games, competitions, etc.) and can even cause a team to fold.

3. When is the first day of practice?
First day of practice is August 1st.

4. What days and times are practices?
During the month of August, practices are Monday - Friday 6:00 pm - 8:30 pm. After Labor Day practices cut down to 3 days a week, 6:00 pm - 8:00 pm.

5. What age does Pop Warner start?
You must be between 5 years old to 15 years old on or before July 31 to participate on a team with Pop Warner.

6. Are there any mandatory dates we need to know about?
The MOST important date for Pop Warner is the LAST Saturday in August, which is the official Pop Warner Certification. This certification is held at the Santa Clara Fairgrounds, and is an all day event.

7. How does volunteer time work?
Since South Valley Pop Warner is run by volunteers, we need as much parent help as possible. All parents are asked to volunteer a minimum of 10 hours per child, and will receive a timecard in August to record their volunteer time. All volunteer time will be confirmed and signed off by the President or Vice President of South Valley. The volunteer time is always set up so that you can watch your child during their game. If you volunteer on the field, it is during their game. If you volunteer in Boosters or the Snack Shack, we always schedule you before your child's game starts so you don't miss the game.
